Payment at the time of service.
We accept:
– Cash
– Credit / Debit cards
– Personal checks
– Returned checks will incur a returned-check fee of $50.00. No further service will be provided until returned check and fee are made good. No personal checks will be accepted will be accepted for 6 months in the future.
– We do not maintain accounts. We do not do periodic billing. You will know the cost of your service before you finish each visit. Lab specimens will not be sent to lab if the appropriate lab fee is not paid.
– We do not maintain any relationship with insurance companies; we do not submit claims to or accept payment from insurance compaines. If you have insurance and want to claim reimbursement, we can provide you with an itemized statement that you can present to your insurance company. We strongly advise you to become familiar with the policies of your insurance company regarding submitting your own claims.
